Born 17th November 1891. Son of Robert And Sarah Ann Cook, of 1, Park Street, Sutton-In-Ashfield, Nottinghamshire. He was a banksman at New Hucknall Colliery. He enlisted at Sutton-In-Ashfield, Nottinghamshire and went to France 2nd March 1915. One of three brothers who served. He died 15th June 1915, age 23.

Buried/Commemorated : Ypres (Menin Gate) Memorial, Belgium. Panel 39 and 41.

His brother Private Harold Cook 1/8th Battalion Sherwood Foresters (Notts & Derbys) Regiment also fell in the Great War.
