Born 12th January 1890 at Sutton-In-Ashfield, Nottinghamshire. Son of Robert And Sarah Ann Cook, of 1, Park Street, Sutton-In-Ashfield ; Husband of Ellen Bradley (Formerly Cook), of 6, Quarry Yard, Sutton-In-Ashfield, He was a miner at Silverhill Colliery prior to enlisting at Newark, Nottinghamshire, 19th August 1914. One of three brothers who served. He died 14th October 1915, age 25.

Buried/Commemorated : Cabaret-Rouge British Cemetery, Souchez, France. Plot 27, Row L, Grave 5.

His brother Private Alfred Cook 1/8th Battalion Sherwood Foresters (Notts & Derbys) Regiment, also fell in the Great War.
